  1. Follow medical instructions
  2. Cancel any appointment at least 24 hours in advance
  3. Refill medication 7 to 10 days before running out
  4. Provide proof of income

The Patient Assistance Program --
helps patients get medications that are not otherwise available.

Our staff works with pharmaceutical companies that donate medications.

Patients provide proof of financial need and communicate with the clinic on a regular basis to refill their prescriptions.

  2. Completed and signed forms
  3. Medical records from the past 5 years
  4. Prescription medication lists

We see patients by appointment only, not on a walk-in basis.
